Wondering if my son has a disorder of some kind?

my son is 4 years old he is a very difficult child. He wines extremly and crys about everything. he stays real nervous acting like when someone is loud or like when he eats if the plate has dividers all the sections must have something in them or he goes into a screaming fit. if you make him a sandwich and you dent the bread or cut it in half he screams its broke or dented an refuses to eat it. He also has alot of trouble being still when he watches tv he usually on his head or spinning or flipping. another thing ive noticed is the way he rings his hands all the time has every since birth. also when he has something on his mind he will repeat it over and over he absolutly never gives up. he has an 11 year old brother who is mildley autistic now i chose not to get genetic testing because this brother has the same dad but a different mom and she has two children who are autistic. ive also read the signs for autism he plays well with his brothers and sisters he doesnt suclude himself he does make eye contact and he speaks very very well. im not sure whats going on with him but something is im concerned and it seems to get worse the older he gets.
